Who we are
A community-based collaborative that addresses vascular health disparities in underserved communities through “pop-up” preventative health screening clinics. Started as a collaboration between UCSF and the Foundation to Advance Vascular Cures, CHAMPIONS is now accessible to institutions nationwide, advancing the mission of equitable and accessible vascular healthcare.
Health Screening and Education
We bring health screening services to local neighborhoods. Our goal is to provide community members with insight into their current health status and to identify those who may be at risk for cardiovascular diseases, including hypertension, diabetes, high cholesterol, and Peripheral Artery Disease (PAD). We offer guidance on connecting with primary care physicians and health insurance.
Together we can work to identify and address disparities in cardiovascular disease.
